FREESURFER VS MANUAL TRACING: DETECTING FUTURE COGNITIVE DECLINE IN HEALTHY OLDER ADULTS AT-RISK FOR ALZHEIMER’S DISEASE Alissa M. Butts, M.S. Marquette University, 2013 Alzheimer’s disease (AD) is a neurodegenerative pathological process that is thought to begin years prior to observable symptom onset. The hippocampus appears to be particularly vulnerable to the underlying brain pathology of AD. Hippocampal volume is a sensitive measure in predicting conversion from mild cognitive impairment to AD, but less is known regarding the use of hippocampal volume in asymptomatic individuals at risk for AD who eventually decline. The inconsistent findings may, in part, be due to the chosen method of hippocampal segmentation. FreeSurfer (FS) and manual tracings (MT) are two common segmentation techniques that have unique costs and benefits. The present study directly compared hippocampal volumes generated by FS and MT in a longitudinal design assessing cognitively healthy elders, with varying degree of risk for AD, over a 4.5-year period. After 4.5 years, 15 participants demonstrated cognitive decline, while 45 remained stable. The results suggest FS consistently produced larger hippocampal volumes than MT, but neither method distinguished between groups at baseline. Longitudinally, individuals who declined experienced a more progressive pattern of atrophy compared to those who remained stable. These data suggest that hippocampal volume over time may be a useful variable in determining cognitive change over time, with the addition of other known risk factors, such as genetic risk. This study also suggests that in presymptomatic individuals, MT may not provide added benefit over the use of the more cost-effective FS.

Effect of amyloid on memory and non-memory decline from preclinical to clinical Alzheimer's disease.

High amyloid has been associated with substantial episodic memory decline over 18 and 36 months in healthy older adults and individuals with mild cognitive impairment. However, the nature and magnitude of amyloid-related memory and non-memory change from the preclinical to the clinical stages of Alzheimer's disease has not been evaluated over the same time interval. Healthy older adults (n = 32...

Comparing manual and automatic segmentation of hippocampal volumes: reliability and validity issues in younger and older brains.

We compared hippocampal volume measures obtained by manual tracing to automatic segmentation with FreeSurfer in 44 younger (20-30 years) and 47 older (60-70 years) adults, each measured with magnetic resonance imaging (MRI) over three successive time points, separated by four months. Retest correlations over time were very high for both manual and FreeSurfer segmentations. With FreeSurfer, corr...

Metabolic syndrome and cognitive decline in early Alzheimer's disease and healthy older adults.

Metabolic syndrome (MetS) is a cluster of risk factors (i.e., abdominal obesity, hypertension, dyslipidemia, glucose and insulin dysregulation) that is associated with cardiovascular disease, diabetes, and dementia. Recent studies addressing the association of MetS with cognitive performance and risk for dementia report mixed results. An important step in clarifying these conflicting results is...

Uncovering the Mechanisms Responsible for Why Language Learning May Promote Healthy Cognitive Aging

One of the great challenges facing humankind in the 21st century is preserving healthy brain function in our aging population. Individuals over 60 are the fastest growing age group in the world, and by 2050, it is estimated that the number of people over the age of 60 will triple. The typical aging process involves cognitive decline related to brain atrophy, especially in frontal brain areas an...
